Copy of Marriage Certificate - married applicants only (4 origianl copies: 1for Dossier and 3 for your Set A –see below) **Your marriage certificate must be notartized in county you were married, and make sure the notary does not expire for at least a year**
Copy of Divorce Decree – single applicants only
Financial Statement (1copy)
Copy of the first page of your Passport (4 copies each; 1 for Dossier & 3 for Set A –see below) *Make sure you sign the passport where required before making copies*
Employment Verification - Must be on company letterhead 1 each
Physician’s Report with HIV test results attached (see attached form) (including all other children) ***You will have to wait on these because they expire after 3 months in Russia. We will tell you when to get these done.
Copy of a valid physician’s license * Again you will wait on this*
Proof of Real Estate Ownership/Rent (see attached form) This is your Warrantee Deed 1copy
Copy of 171-H Form – Immigration Approval This is your approval from INS –you will be waiting for this
Photos of Family and home (see attached form) You will need 4 color copies – 4 pictures to a page. You will need 1 copy for your Dossier and 3 for your Set A.
Name Change Affidavit (see attached form)
Two Reference Letters – single applicants only
Guardianship Affidavit – single applicants only
Police Clearance Report (State or Local) (for all persons living in the house over 16 years of age) This is a licence check. You will need to take your licence to a police station and tell them you are in process of an international adoption and you need a police clearance report. You will need only 1 copy each.
